Insurance Waiver Forms
Entrants in some classes and sections are required to complete insurance waivers in order to compete. Below are the Adult and Minor Waivers for the Caledonia Fair Horse Shows. Please download and complete the appropriate forms and bring them with you to the competition. You can also upload a signed copy when you submit your entries.
